  1. What data will we collect?

In order to operate as a provider of Physiotherapy and other associated medical services we will collect the following information:

Title e.g.Mr, Mrs, Miss, etc.

Name – surname, forename, preferred name etc.

Date of birth

Address

Telephone numbers

Email addresses

Family Doctor/GP/Consultant details

Private medical health insurance policy details

Occupation

Gender

Sport, leisure and pastime activities

Medical history/medical notes

Payment details

Please note that this list will be reviewed from time to time and additions may be made where we realise we have made an oversight or new information is collected.

  1. How will this data be stored?

Currently we collect this data initially on hand written forms which are placed in individual folders. Any written/hard copy correspondence is also placed in these folders. These folders are stored on-site in locked filing cabinets for a total of three years from the previous appointment date. When the file has not been accessed for over three years it is transferred to our archive where it is stored again in locked filing cabinets within locked premises.

Treatment notes (in relation to physiotherapy specific treatments) are stored on our computer server in-house. These are all password protected and a weekly backup is stored on removable disk cartridge at a secure location away from the clinic.

  1. How long will this information be stored?

Clinical records are required to be kept for a minimum of 8 years after the conclusion of treatment or death.

For young persons they need to be kept until the patient’s 25th birthday or 26th birthday if the young person was 17 at the conclusion of treatment, or 8 years after death.

For mentally disordered persons within the meaning of the Mental Health Act, medical records need to be kept until 20 years after the last date of contact between the patient/client/service user and any health/care professional employed by the mental health provider, or 8 years after the death of the patient/client/service user if sooner.

Physio2go Ltd reserves the right to keep data and medical notes for longer than the legal minimum in order to provide a comprehensive background to previous treatment episodes in order to provide a professional level of care. Physio2go Ltd also reserves the right to keep data and medical records for longer than the legal minimum in order to be able to defend itself should a legal case arise where the only reliable record are the notes themselves.

  1. Who will we share your data with?

We may need to write/email to your GP/Consultant/Other medical professionals involved in your treatment/care. We will require your signed request for us to release information to other agencies not directly involved in the delivery of your medical treatment/care.

  1. Will data be used for marketing purposes or sold to a 3rd party?

Physio2go Ltd produces a short electronic newsletter in order to inform patients of the services they provide, clinic news (such as opening times around holidays etc.), staff changes etc. Currently this is produced about 3 times per annum. It is recognised that not everyone is happy to receive this newsletter and we make every effort to avoid sending this once we have been informed that it is not required.

We are a professional Physiotherapy practice and we do not give or sell any of our data to 3rd parties.

  1. Is your data correct?

Physio2go Ltd makes every effort to be accurate and up to date with your personal data. Please contact us if you need to check the accuracy of your data or update e.g. changes of address, telephone numbers, email addresses.

  1. What are my rights?

You have the right to request a copy of your information (no fee) which we will supply within one month of receiving a written request from yourself (not a 3rd party).
